Rural Health Transformation: Building Capacity and Maximizing Impact

With funding now available through the Rural Health Transformation Program (RHTP), states face the critical task of efficiently deploying funds, strengthening healthcare capacity, and maintaining compliance with federal requirements. Our experts have helped clients identify, obtain, and manage more than $95 billion in federal and state grants. Drawing on this experience, we can help states navigate all phases of implementation—maximizing RHTP funding to deliver measurable results and lasting improvements in rural healthcare.

Our Services

We help clients translate funding into results through targeted support across program management, compliance, and oversight.

  • Project Oversight and Coordination: Provide centralized coordination, execution, and oversight support to deploy funds quickly and compliantly.
  • Grant Administration and Compliance: Maintain adherence to Centers for Medicare and Medicaid Services (CMS), 2 CFR 200, and state requirements while proactively managing program risk and adhering to internal controls.
  • Program Development and Management: Design, launch, and administer programs and procurements to support states in distributing funds to subrecipients and vendors.
  • Subrecipient and Vendor Oversight: Assess risk, develop mitigation strategies, monitor subrecipients, and provide technical assistance to ensure effective and compliant delivery.
  • Financial and Performance Management and Reporting: Integrate financial oversight with performance tracking to demonstrate CMS-required outcomes and meet reporting requirements.
  • Stakeholder Engagement: Facilitate transparent communication and alignment with CMS, state leadership, and RHTP stakeholders.
  • Data Analytics and Dashboard Development: Design and implement data systems and interactive dashboards that turn complex data into actionable insights. This includes integrating disparate data sources to track healthcare outcomes, monitor performance, visualize progress toward program goals, and support real-time, data-driven decision-making.

Our Experience

Hagerty is a recognized leader in managing federally funded, state-administered programs—supporting every stage from program design and stand-up to implementation and closeout. Our experience includes supporting a wide range of state agencies with complex grant programs:

Hagerty partners with states to provide comprehensive support across all aspects of Rural Health Transformation Program (RHTP) implementation.

Leveraging Public Health Expertise for Impact

Hagerty combines deep grant management expertise with a strong understanding of public health and healthcare systems. We help states, healthcare organizations, and coalitions translate complex funding and policy requirements into actionable programs that strengthen preparedness, resilience, and sustainable care delivery.

California Emergency Medical Services Authority (EMSA)

Hagerty partnered with California EMSA to develop a Community Paramedicine/Triage to Alternate Destination (CP/TAD) Toolkit that translates policy into practice through standardized protocols, stakeholder-informed design, and scalable implementation resources. Developed through statewide engagement, workshops, and iterative validation with EMS agencies and healthcare partners, the toolkit equips systems to expand care access, reduce avoidable emergency department utilization, and strengthen care coordination.

Granite State Healthcare Coalition (GSHCC)

We supported GSHCC in conducting a Hospital Preparedness Program (HPP)-aligned gap analysis and workforce assessment to identify critical emergency response vulnerabilities across a mixed rural and urban landscape. Through stakeholder engagement, data analysis, and targeted recommendations, the effort produced a prioritized workforce development and training roadmap designed to strengthen readiness and surge capability.

BJC HealthCare

We provide ongoing support to Barnes-Jewish Hospital and BJC HealthCare through staff augmentation for their Emergency Preparedness Program. Hagerty delivers embedded expertise to support training and exercises and maintain critical preparedness plans, including Emergency Operations Plans (EOPs) and Continuity of Operations Plans (COOPs). This sustained support ensures continuous readiness, regulatory alignment, and operational maturity across the healthcare system.
